Intake Care Representative- Evernorth (Infusion/Enteral)

As an Intake Care Representative, you will be a trusted first point of contact for patients, providers, hospitals, and referral partners. This role involves handling inbound calls from patients, helping them navigate the referral and intake process while ensuring accurate information is collected and care can begin without delay. Hours: Monday-Friday 9am-6pm EST, rotating shift (some weekend/holiday coverage 8am-4:30pm EST)

Responsibilities

• Serve as the primary point of contact for patients, responding to calls and providing timely, professional support throughout the intake process.

• Help patients understand next steps, gather required information, and connect them with the appropriate care teams to support a positive patient experience.

• Handle inbound and outbound communication with patients, providers, hospitals, physician offices, and healthcare facilities regarding referrals and care coordination.

• Support timely patient onboarding by gathering, reviewing, and entering referral information accurately into company systems.

• Receive and triage referral packets, medical orders, and supporting documentation to the appropriate teams for processing.

• Coordinate home health services based on patient needs, geographic location, and insurance requirements.

• Verify patient treatment timelines, including first-dose administration and home infusion scheduling needs.

• Upload and maintain accurate patient records and documentation within the medical record system.

• Partner with Intake Coordinators and nurses to ensure referrals move efficiently through the intake process.

• Communicate referral updates and pending items to appropriate team members to help prevent delays in care.

• Facilitate the accurate transfer of insurance information to Insurance Verification teams.

• Deliver exceptional service to internal and external customers while maintaining patient confidentiality.

• Support departmental operations through administrative tasks, phone support, and document distribution as needed.

• Contribute to a collaborative, team-oriented environment focused on quality, accuracy, and patient-centered care.

Qualifications

Required Qualifications

• High school diploma or GED.

• Minimum 1 year of experience in a healthcare, medical office, pharmacy, home health, or related healthcare support environment.

• Knowledge of medical terminology.

• Strong verbal and written communication skills.

• Ability to manage multiple priorities in a fast-paced environment while maintaining attention to detail.

• Proficiency with Microsoft Office applications, including Outlook, Word, and Excel.

• Strong organizational skills and commitment to providing excellent customer service.

• Ability to maintain confidentiality and handle sensitive patient information appropriately.

Preferred Qualifications

• Experience supporting referral management, patient intake, home infusion, specialty pharmacy, or care coordination workflows.

• Experience working with electronic medical records or healthcare management systems.

If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload.

For this position, we anticipate offering an hourly rate of 17.88 - 26 USD / hourly, depending on relevant factors, including experience and geographic location.

This role is also anticipated to be eligible to participate in an annual bonus plan.
