Parallon is an affiliate of Hospital Corporation of America (HCA Holdings, Inc.) and is a leading provider of healthcare business and operational services.  Parallon supports healthcare providers in the areas of revenue cycle (billing, collections, and denial management), technology, patient registration, insurance verification, and health information management.
 
The Incomplete Records Analyst utilizes an electronic medical record system, Horizon Patient Folder (HPF), McKesson Patient Folder (MPF), and EPIC to perform duties. Responsible for analysis, re‐analysis, and management of deficiencies post patient discharge for multiple hospitals to ensure that required physician or mid‐level documentation is present, and that all entries have been properly authenticated before the medical record is filed as complete. This is a Full Time opportunity, working 7 days a week, working every other weekend with days off during the week.  The schedule is based on volume.  Must be flexible, hours run between 5am and Midnight.  During training, employee is expected to work Monday through Friday, from 8:30am to 5:00pm, Eastern Standard Time.

Duties (included but not limited to):

  •  Demonstrates and applies knowledge of typical medical record content and criteria that constitutes documentation deficiencies.
  • Reviews medical record documentation in HPF/MPF, and when appropriate, creates and assigns to the necessary provider(s) dictation, signature and missing text deficiencies for completion according to established policy and procedure.
  • As determined by established work queue processing prioritization, actively manages the various analysis‐ specific work queues to update deficiency information following physician completion or document update activities.
  • Adheres to documented and established workflow guidelines as it relates to adding and re‐assigning accounts to work queues.
  • Identifies document(s) needing to be re‐scanned when image quality is poor and routes the account to the appropriate work queue for resolution processing.
  • Recognizes indexing errors at the document type and patient account levels and routes the account to the appropriate work queue for resolution processing.
  • Addresses physician and other providers with questions related to medical record completion.
  • Maintains or exceeds established productivity and quality standards.
  • Promptly reports issues or trends to the Operations Manager.
  • Performs and adheres to all policies and procedures.
  • Practices and adheres to the Company’s “Code of Conduct” philosophy and’ “Mission
  • and Value Statement”.
  • Other duties as assigned.


  

Qualifications

 EDUCATION

  • High School diploma or GED.

 
EXPERIENCE

  • I year experience in an office environment required, previous HIM experience strongly preferred
